Sioux Center Health has a limited supply of the Johnson and Johnson COVID-19 vaccine left to administer to the community on Friday, May 7 from 11 am – 1 pm in the Family Medicine Clinic.

Vaccination Information:
– Take along your insurance card
– You will have to wait 15 minutes after your vaccine to be observed. You will be directed where to wait after vaccine administration.
– Visit our website to fill out the Johnson and Johnson consent form ahead of time and read the fact sheet for the Johnson and Johnson vaccine.
– Please remember this is a single dose vaccine. No second appointment will be needed.
