Sioux Center Health has received Johnson and Johnson COVID-19 Vaccine from the state that has been allocated for industry and we have 50 doses of the vaccine left to administer to the community.
If you are interested in the vaccine, we are offering a walk-in on a first come, first serve basis on Friday, April 9 from 9 – 11 am in the Family Medicine Clinic.
Vaccination Information:
- Take along your insurance card.
- You will have to wait 15 minutes after your vaccine to be observed. You will be directed where to wait after vaccine administration.
- Please fill out the Johnson and Johnson consent form ahead of time and read the fact sheet for the Johnson and Johnson vaccine.
- Please remember this is a single dose vaccine. No second appointment will be needed.
