Sioux Center Health wants to help people in our communities receive the COVID-19 vaccine. Because the supply is limited, priority groups have been set by the state. As directed by the Iowa Department of Public Health (IDPH), Sioux Center Health anticipates moving from Phase 1A frontline healthcare workers to the Phase 1B priority group.
If you are in Phase 1B, fill out the form below to get on the waiting list. If you need assistance with filling out the form, call (712) 722-8345 Monday – Friday from 9 A.M. – 12 P.M. If you are not in Phase 1B, please do not fill out this form or call at this time until you are eligible. We will update our website and this waiting list form once we are notified that other phases are available and more people are able to be vaccinated from the state. Please only sign up once.
Depending on when the vaccine is available, you will be contacted to schedule your vaccination appointment. For more information on the vaccine, visit here. Please note vaccines are limited and could take months to get through the Phase 1B priority group. Please be patient and wait to be contacted for your scheduled time to be vaccinated.
Phase 1B: Priority Group Includes:
Persons aged 65 years and over.
OR, the following populations vulnerable to high risk of exposure or severity of illness (listed in order of priority):
• First Responders
• PK-12 staff, early childhood education, childcare workers
• Frontline essential workers in food, agriculture, and manufacturing sectors who live or work in non-social distanced settings
• Individuals with disabilities living in home settings and their direct care staff
• Staff and individuals living in congregate settings not covered by previous Phase or Tier
• Government officials, including staff, engaged in business at the State Capitol
• Inspectors responsible for health, life and safety
• Correctional facility staff and individuals incarcerated
Vaccination Appointment Information:
Appointments are required and you will be contacted with your scheduled date and time as vaccines become available. Please bring your insurance information as there will be an administration fee and print and fill out the consent form. There will be no out-of-pocket costs for receiving the vaccine. Private insurance companies and government insurance programs like Medicare will full cover the cost of the vaccine. For those uninsured, it will be fully covered. Those vaccinated will be required to remain for 15 minutes of observation after receiving the vaccine, with 30 minutes of observation being recommended for patients who have experienced a severe allergic reaction in the past, such as to oral medications, foods, bees, etc.
While we understand the desire for many to receive this vaccine, we ask that you be patient as we work through this process. Please do not overwhelm the phone line with general inquiries. We will update our communities of additional steps in the vaccination process through our website, channel 12, KSOU, and our facebook page. Because of the volume of people to be vaccinated and stringent guidelines, Sioux Center Health will not be vaccinating any group early or keeping a waiting list for those wishing to be vaccinated outside of the Phase 1B priority group. All vaccinations will be given in a vaccination clinic setting and not on an individual basis. All clinic dates are based on adequate vaccine supplies, but dates could change if supplies are not received in a timely manner.
