Memory Care

The Siouxland Memory Center is a place where individuals and families can find the support they need, along with clear answers and compassionate care. Clinics are held weekly on Wednesdays with Dr. David Janssen and Morgan Meissner, PA-C.

Our memory support offers families touched by Alzheimer's disease and other dementias confidence that their loved ones will be engaged and safe every day. 

Many factors influence memory loss.

The key to an accurate diagnosis is a thorough and complete diagnostic evaluation. The Siouxland Memory Center provides each patient with a comprehensive assessment including:

  • Cognitive examination
  • Depression screening
  • Diagnostic imaging (if indicated)
  • Functional and sensory exam
  • In-depth medical history
  • Patient and family education
  • Psychological assessment
  • Targeted physical and laboratory exam

An accurate, early diagnosis is the first step toward effective treatment and care. In addition to diagnostic services, the Siouxland Memory Center offers education and support for patients, caregivers, healthcare professionals, and long-term care providers.

During your initial evaluation, we conduct an in-depth patient history, along with several tests. In many other facilities, these necessary tests might be skipped because of time constraints, but at the Siouxland Memory Center, we spend approximately two hours with each patient during the initial evaluation.

The team then evaluates the findings, makes a diagnosis (if indicated), and designs a personalized treatment plan. The patient will need to return for an initial follow-up after two weeks to discuss this plan. Additional follow-up visits are usually recommended at six- or twelve-month intervals. Patients will have the assurance of meeting with a team that knows their case and tracks their progress.

Memory Café Caregiver Support Group

Sioux Center Health is proud to offer Memory Café Caregiver Support Group, a program designed to bring together individuals with any form of dementia or memory loss and their care partners. Caregivers will meet with a social worker in a support group environment and the persons living with dementia will engage in meaningful activity. Activities could include arts/crafts, music, movement, reminiscing, and more. This time will be facilitated by trained nursing, social work, and life enrichment staff. Meeting the third Monday of each month from 10:30 - 11:30 a.m.

If you are a caregiver, please know that you are welcome to attend on your own—there is no requirement to bring your loved one with you.
