Our History

Over 70 years ago, a group of local leaders from Sioux Center envisioned a community hospital to serve the health needs of the community. In 1951, that vision became a reality when the new hospital building was constructed and its doors were opened.

Community Health Needs Assessment (CHNA)

The Community Health Needs Assessment (CHNA) is a federal requirement for 501(c)(3) hospitals, mandated once every three taxable years. The information must be disclosed in the hospital’s annual information return to the IRS (i.e., Form 990 and related schedules).

While the CHNA is a federal requirement, we must remember that assessing our communities and providing community benefit programs is more than just a nonprofit obligation. Conducting a needs assessment and responding to community health needs allows us to live our mission of bringing hope, health, and healing to life — and to make a meaningful difference in the communities we serve.
