To Schedule Your Appointment
We recommend calling ahead whenever possible to make an appointment. You can also request an appointment through AveraChart. If you need to be seen the same day and your regular healthcare provider is not available, we will offer you an appointment with one of our other providers.

For your convenience, we recommend completing a new patient registration form and patient history form. Please complete the forms prior to your appointment. We will also need to scan both your medical insurance card and driver’s license.
