Sioux Center Health provides comprehensive annual wellness exams designed to support your long-term health. Most insurance plans completely cover these preventive visits, emphasizing the importance of early detection and proactive care.
We also offer sports physicals to ensure athletes are healthy and ready for the season.
Now is a good time to get your family’s sports physicals and preventative annual wellness exams scheduled! Sioux Center Health offers a comprehensive head-to-toe evaluation of your child’s current health status and review of immunizations during their appointment.
ATTENTION: Students are required to have their first dose of a meningitis vaccine prior to entry into 7th grade and a second dose prior to entry into 12th grade. See our Immunizations page for more information.

The Annual Wellness Exam Includes:
- A complete head-to-toe physical
- A complete set of vital signs including height, weight, BMI, heart rate, respiratory rate, and blood pressure
- A review of your child’s allergies
- A review of your child’s medications
- A review of your child’s immunizations, along with receiving any immunizations that are due
- A review of your child’s past medical history
- A review of your child’s social history, which includes risky behaviors such as tobacco usage, choices in foods and caffeine use, alcohol use, and seat belt use to name a few
Most importantly, this is an excellent opportunity for your provider to educate your teen about healthy lifestyles and behaviors.
The healthcare industry recognizes the benefits of prevention and most insurances will cover one visit annually with no co-pay required! We encourage you to check your insurance policy for your coverage benefits to see if it covers one per calendar year or if it needs to be 366 days prior to calling to schedule. If your child is not covered by an insurance policy and needs an athletic physical, please contact us for options.
You can schedule a preventative annual wellness exam at any time. We highly encourage you to schedule sooner rather than later as students who participate in fall sports will need this completed PRIOR to the first day of practice.
Please print and complete (ensuring it is signed by a parent or guardian) the Sport Physical Questionnaire and bring it to the athletic physical appointment.
